Following admission, you will be asked to complete an End of Life wishes form (Advanced Care Directive).
An Advance Care Directive which is sometimes referred to as a ‘Living Will’ is a legal form which should provide a clear statement that sets out your directions including your wishes and values that need to be considered before medical treatment decisions are made on your behalf.
It is important for the residents that dignity is maintained and residents’ wishes are followed to the end. Care facility staff would like to know if you have any special needs/wishes/cultural requirements regarding Palliative Care.
Our Palliative Care Officer or CNM is available to discuss your questions or particular requirements and support you during this time.
Staff have regular Palliative Care internal and external training and updates.
